Abstract

This article reflects on five disasters - the explosion at the Pike River coal mine on the West Coast on 19 Nov 2010; the capsizing of the 'Herald of Free Enterprise' ferry between Dover and Bruges-Zeebrugge on 8 Mar 1987; the Union Carbide factory explosion in Bhopal, India, 2-3 Dec 1984; the crash of Turkish Airlines flight 981 on 3 Mar 1974; and the explosion on the Piper Alpha oil/gas rig in the North Sea, 6 Jul 1988. It discusses the failures in information management that contributed to each disaster.

Author Biography

Janita Stuart

Janita Stuart completed her BSc and MLIS degrees in the USA. She began her career as a librarian gaining wide ranging experience in reference service, customised client services and IT library systems. Over time she branched out into Web development, records and archives management, and Knowledge and Information management. She completed the MIM and Nat Dip Business Management in New Zealand. She gained expertise in strategic planning, and training and development of personnel. She has authored a book and numerous articles arising from her professional pursuits and as a conference presenter. Based in Wellington, she is Information Manager for WorkSafe New Zealand.
